MDS with Multilineage Dysplasia and Ringed Sideroblast

MDS with ring sideroblast is a MDS characterized by features of cytopenias, dysplasia and ring sideroblasts. It is commonly associated with SF3B1 mutation. Further classification is dependent of number of lineages with cytopenia and dysplasia.

Blood Film Features:

  • Cytopenias with dysplasia – (see MDS overview page for specific features)
  • Prominent features may be a dimorphic blood picture with macrocytes and a small population of microcytes

Bone Marrow Features:

  • Bone marrow usually normocellular to hypercellular
  • Erythroid hyperplasia
  • Dysplastic features (see MDS overview page for specific features)
  • Ring Sideroblast:≥ 15% (or ≥ 5% if SF3B1 mutation has been documented)
  • To be considered a ring sideroblast: ³ 5granules, encircling >1/3 of the nucleus
